A small, single-dose of azithromycin (Zithromax) in infants at risk for severe bronchospasm has been shown to prevent bronchospasm in the early stages of illness and improve quality of life.
This retrospective study compared the clinical efficacy of two single-dose oral therapies - azithromycin 500 mg and azithromycin 10 mg - in infants with severe bronchospasm or persistent respiratory distress.
The study was conducted at the Clinical Research Center of the School of Medicine, Wrocław, Poland.
Study design: Prospective, single-dose, randomized, double-blind, controlled clinical trial. Patients with respiratory distress (R& D≥>3%) and severe bronchospasm (R& D≥>4%) were included if their infants (10 mg/kg) were clinically significant for severe respiratory distress at 2 or more weeks of age. The trial was conducted at the Clinical Research Center of the School of Medicine, Wrocław, Poland.
For the analysis of the clinical efficacy of azithromycin in infants at risk for severe bronchospasm, patients with a clinical R& D≥>3% were excluded. A total of 1346 infants were included in the study. Of these, 971 (9.7%) infants were hospitalized. The most frequently used medications were clarithromycin and fluconazole. The oral therapy regimen was similar in all groups: azithromycin 500 mg, 10 mg/kg, and azithromycin 10 mg/kg, and the regimen was similar in all groups: azithromycin 500 mg, 10 mg/kg and azithromycin 10 mg/kg, respectively.
The primary end point was severity of bronchospasm (R& D≥>4%) or respiratory distress (R& D≥>3%) and the secondary end points were the occurrence of other adverse events.
Statistical analysis of data was performed using SPSS version 17.0 (SPSS Inc, Chicago, IL, USA). Continuous variables were expressed as mean ± SD or median and interquartile range (IQR) for continuous variables and percentages for proportions. The categorical variables were expressed as absolute and relative frequencies and proportions. Continuous variables were compared using Chi square test. P values below 0.05 were considered statistically significant. Kaplan-Meier survival analysis was used for statistical comparisons of survival.
A total of 469 infants (3.7%) were hospitalized in the study. The mean age at first diagnosis was 4.6 ± 1.3 months in the azithromycin group (mean ± SD age 2.4 ± 1.4 months), and the median age at first diagnosis was 28.5 ± 2.3 months. At the time of first diagnosis, mean age was 5.8 ± 1.1 years in the azithromycin group (mean ± SD age 5.3 ± 1.3 years) and the median age was 29.0 ± 1.3 months. The mean duration of bronchospasm was 13.6 ± 1.3 days in the azithromycin group (mean ± SD age 11.0 ± 2.1 days) and 15.2 ± 1.1 days in the azithromycin group (mean ± SD age 12.1 ± 2.0 days).
The mean duration of bronchospasm was 8.0 ± 1.6 days in the azithromycin group (mean ± SD age 4.9 ± 1.2 days) and 14.4 ± 2.0 days in the azithromycin group (mean ± SD age 11.4 ± 2.4 days) (p=0.000). There were no significant differences between the groups in the duration of bronchospasm or respiratory distress (p=0.844 and p=0.074 respectively). The incidence of other adverse events in the azithromycin group were similar to the incidence in the other groups.
Patients with a clinical R& D≥>3% were also excluded. The mean duration of bronchospasm was 4.7 ± 1.
